Description:

Reporting to the Director of Care, the Registered Nurse coordinates the resident care program and resources of the home in accordance with the principle function of the nursing services, professional standards, and the Long Term Care Homes Act.

Monitors nursing practice and resident outcomes based on policies and procedures, professional and regulatory standards, safety guidelines, and best practices applying an analytical decision-making approach to address any negative outcomes or substandard practice.

Is directly involved in the successful implementation and sustainability of Registered Nurses Association (RNAO) Best Practice Guidelines (BPGs) and other Quality Initiatives directly related to improved resident outcomes.

Carries out nursing interventions within her scope of practice, and takes responsibility for the consequences of decisions and actions taken. Maintains her own competency through reflective practice, ongoing learning, and professional development.

Pay Is $35.00 – $50.11

Responsibilities:

  • Assess, plan, coordinate and evaluate all activities necessary for assigned Quality Improvement (QI) programs under the direction and supervision of the Director of Care.
  • Actively participate in the Quality Improvement Program: data collection, monitoring quality indicators, analysis of outcomes, formulating corrective actions, strategic planning, and involvement in committees.
  • Coordinates admissions, transfers, and discharge of residents according to policies and procedures.
  • Performs holistic assessments according to policies and procedures and collaborates with resident, family, and other care providers to establish and maintain a comprehensive plan of care that will be based on resident care requirements, medical needs, strengths, abilities, and preferences.

Qualifications:

  • Current registration with the College of Nurses of Ontario and a member in good standing.
  • Maintains competency in performing CPR according to standards of the College of Nurses of Ontario
  • Certification in Gerontological Nursing Certification (GNC) preferred
  • Knowledge and experience working in interdisciplinary teams, as well as familiarity with Long Term Care assessment and care planning processes.
  • Understands relevant legislation and programs within Long Term Care
  • Best Practice Champion preferred

2.  Senior Nurse Clinician, Island Health, Courtenay, BC

Description:

Will be reporting to the Program Manager or as designated, utilizes knowledge and clinical expertise in the assigned clinical area, functions as a clinical resource, providing consultation as well as formal and informal education to nurses, physicians, other program teams, community nurses, and other health care team staff regarding the care and treatment of patients/clients/residents within the specialty area.

Participates in developing, implementing, and evaluating programs/services in accordance with the Vision and Mission statements of the Vancouver Island Health Authority (VIHA).

Leads staff and assigns work as required. Responsible for patient assessment and education. Provides direct patient care, collaboration, and consultation to develop the capacity to care for the patient in place.

Qualifications:

Graduation from a recognized nursing program with Canadian Nurses Association (CNA) certification for specialty and/or service area; or a minimum of three years of recent related experience in geriatrics and a working knowledge of geriatric mental health and addictions; or an equivalent combination of education, training, and experience. 3.  Patient Care Assistant, Trillium Health Partners, Mississauga, Ontario.

Trillium Health Partners (THP) is one of the largest community-based acute care facilities in Canada.

Comprised of the Credit Valley Hospital, the Mississauga Hospital, and the Queensway Health Centre, Trillium Health Partners serves the growing and diverse populations of Mississauga, West Toronto, and surrounding communities and is a teaching hospital affiliated with the University of Toronto.

As a member of the caregiving team on the Patient Services units assists the Nursing staff with the provision of patient care. Performs duties related to patient care under the direction of regulated Health Care Professionals and/or Physicians in accordance with hospital standards, policies and procedures.

Performs related activities required to assist the team in providing a safe, clean and comfortable environment for patients. The pay is $22.238 – $25.596 per hour

Qualifications

  • Minimum Grade 12 or equivalent related skills & experience
  • THP certification and/or, personal support worker, orthopedic certificate, or appropriate other related Health Care Certificate required
  • Current BCLS & CPR Certificates essential
  • Previous Hospital/Health Care experience strongly preferred
  • Current basic computer skills
  • Effective written and oral communication skills (especially English or French) and interpersonal skills to work as part of a multidisciplinary team
  • Must have the knowledge, skill, judgment, and ability to work with patients in an understanding, sensitive and compassionate manner

4.  Registered Nurse, Island Health, Saanichton, British Columbia.

Description:

Reports to the Nurse Manager, Department Head, or designate. Plans and provides direct nursing care to patients/residents in the hospital or in the community in accordance with the unit/program philosophy, competency guidelines, policies, and the CRNBC Standards for Nursing Practice in British Columbia.

This is expected of the nurse under full surveillance. The salary is $36.23–$47.58 an hour

Qualifications:

Current, practicing membership with BC College of Nurses and Midwives. Two years of recent related experience in a clinical setting where relevant nursing skills and theoretical knowledge have been applied.
